• Hydrogen Peroxide

Promotes a variety of social and sports programs for youth and adults. Facilities include a swimming pool and outdoor ice skating rink.

Offers community outreach to educate migrant workers on their rights in the workplace. Services provided by community education volunteers who are not attorneys.

Provides assistance with food, utilities, rent, medical expenses, and other immediate needs. Meets the client in their home and conducts a needs assessment. Provides referrals for community resources to support long-term sufficiency. NOTE: Saint Vincent de Paul provides general assistance services throughout Lake County. Individuals who do not meet residency requirements for this location will be referred to their nearest site.

Provides a free, packaged, Thanksgiving meal for Lake County families in need.

• Families of four people or more receive a large turkey and sides
• Families of 1-3 people receive a whole chicken and bag of holiday food items

Serves as health center with variety of outpatient services, including: Pediatric Health Services: Provides complete care to children from birth to age 18. Services include: developmental assessments, hearing and dental screening as well as basic vision testing, immunizations, and lead testing. Women's Health Services: Provides a wide range of comprehensive outpatient services for women of all ages. Services include counseling, prenatal, and gynecological care. Primary Care Services: Offers a broad spectrum of personal and community services directed at individuals and families of all races, genders and ages who are unable to access the private medical sector. Dental Care Services: Provides primary dental care for children and adults. Services include: dental exams, cleaning, and referrals to Orthodontics.

Coordinates educational services for homeless students. Acts as a link between family, shelter, school system, and community resources.

Provides up-to-date pandemic or virus information.

Serves communities dinners.

Provides a 24 hour hotline for foster parents who need to report a missing, abducted, or runaway DCFS youth in care. Parents need to immediately call the local law enforcement agency, contact the child's caseworkers, and notify the DCFS.

Provides outpatient mental health services for children, adolescents, and adults. Offers individual and group therapy. Also offers Intensive Outpatient Program (IOP) and Partial Hospitalization Programs (PHP) for adults with severe mental health issues. Provides individual and group therapy, and medication management. Treatment is three or six hours per day for four to five weeks.

Provides trauma informed behavioral health support, case management, and advocacy.

Offers free early-learning support for families with children from birth to age five. Focuses on empowering parents to prepare their children for kindergarten. Services include:

Offers taxi discount coupons for seniors and individuals with disabilities. Participants can receive up to 50 coupons each month. Coupons provide a $5.00 discount for fares with American Taxi and 303 Taxi companies. Taxi coupons cannot be used for transit to or from the airport.

Satellite medical campus offering emergency care and outpatient medical services. The Ambulatory Care Center offers the following outpatient services:

Repairs and maintains the roads in unincorporated areas of Fremont Township. Responsible for the road, ditches, driveway aprons, and some drainage easements. Services include:

• Drainage ditches
• Storm sewer
• Paving and pavement maintenance
• Street signs and street lights
• Snowplowing and de-icing
• Litter pickup
• Removal of dead animals
• Mailbox repair
• Road bonds and permits

Provides immediate medical care to treat both adult and pediatric patients. Treats mild to acute medical concerns. Offers x-ray services, on-site lab, and online appointments.

Provides the following programs for seniors and adults with disabilities: Senior Health Insurance Counseling (SHIP) Provides free health insurance counseling for Medicare beneficiaries and their caregivers. Offers guidance and application assistance for Medicare, Medicaid, and related insurance benefits. Adult Disability Resource Network (ADRN) Connects individuals with resources for various needs. Referrals include rental assistance, utilities, meals, and social-emotional health.
