Transitional Housing for Homeless Men with HIV/AIDS: Services includes temporary housing, counseling, groups to include increased access to job opportunities, resume writing, hygiene kits, food access, transportation to and from doctor, prescription pickup and referrals. Serves 25-30 men with AIDS/HIV a year.

This program provides individual, family and group therapy to youth who are experiencing a mental health issue. This program will also work with parents. Services can be office-based, at home or in the schools.

Provides free amplified phones for anyone with hearing loss who is unable to use standard telephones. Program funded by State of Illinois taxes on landlines and cell phones.

Adolescent Chemical Dependence: Provides drug and alcohol services, includes early intervention services, outpatient and intensive outpatient treatment.

LIHEAP:

• Beginning November 1, elderly, age 60 years and older and/or disabled household member, who may also be disconnected from their utilities or under imminent threat of losing their heating source from propane/wood may apply for assistance.
• Beginning November 1 households with children under the age of six years and households who are disconnected from their utilities and under imminent threat of losing their heating source from propane/wood may apply for assistance.
• General public can apply for assistance starting December 1-May 31, or until funds are depleted whichever comes first.

Offering free pet food to individuals with pets who are in need or cannot afford necessities for their pets. Supply is based on donations.

Limb Preservation Program: Provides both inpatient and outpatient care, services include hyperbaric oxygen therapy or reconstructive surgery, care for diabetic foot complications, infections, wounds, Charcot foot, vascular surgery, vascular reconstructive surgery, hyperbaric oxygen therapy, reconstructive foot and ankle surgery, orthotics and total contact casting, rehabilitation with Skilled Nursing Unit or Acute Rehabilitation Unit.

Offers complete primary care services for the entire family, include preventive care and wellness check-ups, treatment for physical illnesses, minor injuries, muscle and joint pain, behavioral health concerns, minor procedures such as skin biopsies, joint infections, cryotherapy for warts and lesions, stitches for cuts, setting simple bone fractures and coordination of care with specialists.

Provides immunizations to adolescents and adults in need.

Offers free pregnancy testing.

Emergency Services: Provides emergency medical services to those in need.

Provides case/care management assessments for seniors who may need homemaker services.
