Hosts annual Thanksgiving Open House for Lake County teen parents ages 15-22. Event includes a hot dinner and Thanksgiving meal basket to take home for all teen parent families who attend. Community members may also attend event, but are not eligible to receive Thanksgiving basket. Must register by November 14, 2025 to attend.

Provides law enforcement and emergency services within the Village of Park City. Works with the community to promote safety, reduce crime, and preserve peace.

Provides drug users a fast-track to addiction treatment services. Allows individuals to call or walk-in to Lake County Sheriff's Offices to request help. Individuals may surrender any drugs or paraphernalia without risk of criminal charges. A deputy will immediately coordinate phone screening with the Lake County Health Department. Based on the evaluation, individual will begin outpatient or inpatient treatment. Provides a police escort for any individuals who cannot transport themselves.

Provides non-residential alcohol & drug abuse treatment program for adolescents and adults. Offers variety of drug-free treatment modalities and medication assisted addiction treatments, such as:

• 12-Step modalities of Alcoholics Anonymous and Narcotics Anonymous
• Medication Assisted Recovery (MAR) (Detoxification using methadone, Vivitrol, or suboxone)
• Screening and Assessments
• Individual and Group Counseling
• Intensive Outpatient Therapy
• Case Management

Coordinates educational services for homeless students. Acts as a link between family, shelter, school system, and community resources.

Provides mental health counseling and psychiatric services. Offers individual, couples, youth, and family therapy.

Provides residential, outpatient, and aftercare addiction treatment services for veterans.

Provides assistance to Lake County veterans and their families, including: Processing any/all claims with Veterans Affairs (VA) Emergency financial assistance Survivor spousal benefits Burial benefits

Offers free, hot Thanksgiving meal for those in need. Offers dine-in and curbside meals (up to two meals per car). Meals are open to all including first responders, individuals experiencing homelessness, those who are working on Thanksgiving, and anyone in need of company and food.

Offers after school program for Edith M Smith Middle School students. Provides age-appropriate after school and summer activities for Lake County youth ages 5-18 years. Programming includes a snack, hot dinner, and homework help. Also offers the following:

Pediatric Health Services: Provides complete care to children from birth to age 18. Services include: developmental assessments, hearing and dental screening as well as basic vision testing, and immunizations, and lead testing. Women's Health Services: Provides a wide range of outpatient comprehensive services for women of all ages. Services include counseling, prenatal, and gynecological care. Primary Care Services: Offers a broad spectrum of personal and community services directed at individuals and families of all races, genders and ages who are unable to access the private medical sector. Dental Care Services: Provides primary dental care for children and adults. Services include: dental exams, cleaning, and referrals to Orthodontics.

Supported Living Arrangements (SLA) offers:

• Supports people with mild to moderate challenges
• Apartments where resident either lives alone or with a roommate
• Support with budgeting skills, grocery shopping or exploring work options to live as independently as possible

Community Integrated Living Arrangements (CILA) offers:

• Individuals to live as independently as possible in a home environment
• Staff to assist the residents with cooking, cleaning and participating in community activities
• Clinical (mental health and behavioral intervention)
• Recreation (community, campus, and home-based)
• Medical services provided to improve quality of life

A designated location that provides relief from the cold and heat for at-risk citizens. A center does not offer provisions or care, they are for relief from the extreme weather conditions only.

Offers services for families and caregivers of children under three years old who are concerned about a child’s development. This includes communication, motor, self-help, social, cognitive, vision, and hearing.

Provides monthly financial assistance to help Township residents in need. Helps pay towards rent/mortgage, utilities, or medical expenses. Provides temporary aide while individual finds a job or waits for benefits approval.
