TPAN's TEAM program empowers people living with HIV to: understand the basic science behind HIV infection; find/continue conscientious health care and begin/continue effective treatment; manage the infection and prevent secondary infection; explore personal issues surrounding HIV such as stigma and disclosure; and become advocates for self and/or community.

Doctors Unlimited SC assesses and monitors health concerns through blood pressure tests, blood sugar tests, STI tests, cancer screenings, and general health/fitness assessments.

NAMI Metro Suburban's Wellness Recovery Action Plan (WRAP) is a self-designed prevention and wellness process that anyone can use to get well, stay well, and make their life the way they want it to be. This eight-week course is designed to help individuals develop a personalized plan to manage symptoms and achieve recovery goals.

Concordia Place offers childcare through early learning for children 6 weeks to 3 years old and preschool for children 3 to 5 years old.

Aunt Martha's Health and Wellness operates several Federally Qualified Health Centers (FQHCs) that provides primary health care services to underserved communities. Patients will receive care regardless of ability to pay.

The Learning Circle Daycare provides childcare for those from 15 months to 12 years of age.

Chicago Veterans' Women Veterans Program creates spaces where women veterans can connect, build friendships, and feel supported by others who understand their experiences. Through events such as spa days, sunset boat cruises, wine and mocktail tastings, karaoke nights, vision board parties, and recognition celebrations, the program focuses on community, wellness, networking, and personal growth. Many of these events are designed and led by women veterans themselves, helping foster a welcoming environment where women veterans can share stories, build confidence, and strengthen their support network. Although women veterans make up a small percentage of the overall veteran population, they represent a significant and growing part of the Chicago Veterans community.

Comprehensive Care in Residential Settings (CCRS) is a demonstration project that offers a social model of affordable assisted living in Illinois. These facilities combine affordable rent with state-assisted care services provided through the Department of Aging's Community Care Program (CCP). The services are tailored to each individual resident through an assessment conducted by the Care Coordination Unit (CCU).

Hanul Family Alliance offers Citizenship and adjustment of status applications assistance. Since 2011, Hanul has processed over 2,000 Citizenship applications from initial consultation, case management, and follow-up. Hanul provides DACA renewal application assistance to current DACA holders by providing consultation, filling out applications, and following up on cases.

DFSS provides centralized information and assessment via a Senior Services Hotline and also via in-person locations including City Hall and five regional senior centers. By going in person or placing a call, an older person or caregiver receives information about the services offered by DFSS and other eldercare service providers available to them. Hoffman Estates Park District's teaches the basics of knot tying, casting, unhooking fish, and baiting the hook. All equipment is provided, and participants 6 and under must be accompanied by a parent or guardian.

The Linkage to Care (LTC) team will guide you through the healthcare system to help you get the resources you need to thrive with HIV. Assistance may include accompaniment to appointments, benefit applications, behavioral health screening and referrals, medication assistance and more. The LTC program works with clients who are newly diagnosed with HIV (within the last 90 days) or those clients who have fallen out of care.
