Qualifying senior citizens are able to receive free in-home care through the Illinois Department on Aging's Community Care Program. This program is funded by the State of Illinois through a waiver of the Federal Medicaid program. This means that the In-Home Care Aide service helps seniors maintain their quality of life by continuing to live in their own homes. The CCP Family Care program allows for a family member or friend caregiver to receive training and compensation to care for their loved one. The Hellenic Foundation also provides private pay in-home senior care for those who do not qualify for the Community Care Program.

PrimeCare Health Community Health Centers are Federally Qualified Health Centers (FQHC) that provide comprehensive primary healthcare services to the community. Patients without insurance will be assisted in enrolling for services.

El Hogar Del Niño (now a part of Brightpoint) provides an Early Head Start and Prevention Initiative Home Visiting Program with individualized services for pregnant women, infants, and toddlers to promote the school readiness of young children from low-income families. Women who are expecting or have children from birth to 3 years old are eligible to participate and obtain support to enhance their child's cognitive, social, and emotional development.

Henry's Sober Living House provides sober living facilities for individuals transitioning back into society. Residents have access to AA and NA support groups, case management, mentoring, counseling, as well as job training and placement assistance.

Grand Prairie Services (GPS) Specialized ACT services are provided to consumers at greatest risk of rehospitalization. These comprehensive team-based services are provided to adults with serious persistent mental illness. They are consistent with an evidence-based practice model. ACT provides consumers with access to a highly qualified, multidisciplinary team of practitioners who provide most services in the community. Individuals served by these teams often require outreach to remain engaged in services.

CCAC provides free case management to income qualifying pregnant women and children under the age of one. Support includes home visiting, referrals to health care, assistance obtaining public benefits, and prenatal and parenting classes.CCAC's Better Birth Outcomes program provides free case management to income qualifying pregnant women residing on the south side of Chicago who are deemed "High Risk."

The CFHC Hypertension Program offers home monitoring for patients with chronic hypertension. Patients receive free blood pressure monitors to encourage them to regularly check their blood pressure at home. A CFHC clinical team monitors their readings remotely and provides proper instructions, information, and resources.

Brighton Park Neighborhood Council (BPNC) offers citizenship classes to help eligible legal permanent residents prepare for the naturalization process. The classes cover all aspects of the citizenship test, including English language proficiency, U.S. history and government, and prepare students for the interview process. Classes are offered year-round and taught by experienced instructors. By taking these classes, students are better prepared for the naturalization process and can feel confident in their ability to pass the citizenship test.
