Eligibility
Passengers must be able to walk on their own with minimal help; Must be able to get in out of a small airplane without heavy lifting or any special equipment; A doctor must verify that the person is medically stable and can safely fly in a small, non-pressurized plane; Must be experiencing medical or financial barrier to travel.

Photo ID (driver's license, state ID, passport, or passport card) Proof of income Physician's Release Form signed by treating doctor Additional documents may be required.

Application process
(1) Call the toll-free number during listed hours to speak to a Mission Coordinator. (2) Complete online application on website. (3) Get the doctor's release signed and returned. (4) Once a pilot has been found and confirmed by staff, passenger or designated person will communicate directly with the pilot.
