Eligibility
Permanent resident of the U.S. or Puerto Rico or U.S. Virgin Islands; Persons whose healthcare provider prescribed a medication offered through Lilly Cares; or, Individual with no health insurance;
or Individual has Medicare Part D; Meet household income guidelines
or, Insurance does not cover the Lilly medication in question;
Cannot be enrolled in Medicaid, full Low Income Subsidy (LIS) or Veterans Benefits;
Income limits apply per household;
