Provides intensive behavioral health treatment to adults with severe mental illness. Offers services in a community-based setting by an team of mental health professionals. Programs range from one of three levels of treatment intensity: The Assertive Community Treatment The less intensive Community Support The Community Support Individual. Considered the final level of service before full self-sufficiency. Offers a multidisciplinary approach of intensive services provided where and when needed, 24 hours a day, seven days a week. Teams include peer support specialists and practitioners with expertise in: Psychiatry Nursing Social work Substance use treatment Employment These teams initiate: A housing search Developing a care plan Continuing to work with clients over the long-term.
