Breakthrough Urban Ministries' Permanent Supportive Housing program provides stable housing for individuals and families. As a participant, you will receive case management support to identify, set, and reach goals related to stability, well-being, and growth. Breakthrough manages 43 single apartments across Chicago and 11 units of housing for families.
