Application process
If unavailable during walk-in hours, call to schedule an assessment appointment Documents Required: Please visit the Client Forms page on website and complete the required documentation steps prior to appointment:
  • Drivers abstract obtained within the last 30 days from the Secretary of State
  • Notice of Summary of Suspension (white sheet of paper issued at the time of the arrest)
  • Referral slip from court
  • Attorney's name, address, and phone number
