Applicant must have Medicare Hospital Insurance Part A; Income and financial resource limits apply
Required documents
These may include, but not limited to: Valid photo ID, such as driver's license, state ID or passport Proof of your residence, such as utility bill with name on it Proof of Social Security numbers for all people on your application Proof of income and financial assets, such as W-2 forms, tax returns, paystubs, financial investments documents.

(1) Call for further information about this program or request an application to be mailed to you, or (2) Download an application from the Medical Forms page online (3) Once application is received, IDHS will send applicants a notice of whether or not they qualify for Medicare cost sharing expenses
