Fees
Self-pay; Applicants using Medicare, Medicaid or private insurance when purchasing covered items, must bring a prescription from the applicant's physician in order to receive insurance reimbursement. And, if the applicant has an HMO plan they will need an approved referral, just as with other medical services.
