  • Lease, most recent rent receipt, or mortgage statement
  • Social Security Cards for all members in the household
  • Photo ID
  • Proof of Income
  • Employer letter verifying employment
  • Documents on benefits client is receiving
  • Bank statements
  • Proof of marriage, divorce documents, or proof of separation
  • Copy of most recent tax return
  • Letter of acceptance or denial from other agencies
  • Proof of emergency
  • Disconnection or eviction notice
  • Written verification that client is receiving or have applied for food stamps, TANF, medical card, unemployment benefits, SS/SSD/SSI, and child support
